Form due at 1st membership meeting (October):

  1. Robinson Crew Booster Club Membership Application - submit to membership chairperson
     

Forms due at 2nd membership meeting (November) & must be submitted prior to winter training:

  1. FCPS Emergency Care Information - submit 4 copies to membership chairperson.  This form contains text boxes so all information except for signatures can be typed in and then printed but you will not be able to save it with the text unless you own a copy of Adobe Acrobat.

  2. VHSL Athletic Participation/Parental Consent/Physical Examination Form-submit to the Robinson Athletic department (KEEP A COPY for your records!)

  3. Robinson Secondary School Athletic Participation Policy Acknowledgement - submit to membership chairperson
     

Forms due at 3rd membership meeting (January) & must be submitted prior to spring training:

  1. FCPS Parental Authorization and Acknowledgement of Risk for Field Trip - must be signed by BOTH parent & rower.

  2. FCPS Field Trip Driver's License & Vehicle Insurance Information - must be completed for (at least) one parent of each rower AND a separate form completed for the individual rower if they will be driving themselves.  Please complete both insurance and driver's license information or the form is not valid.

  3. Robinson Secondary Parental Authorization & Acknowledgement of Risk -- must be signed by BOTH parent and rower.
     

On-line waiver (2009) must be submitted prior to midnight on Thursday, March 19th:

  1. Members who do not have a waiver on file by that date will NOT be able to participate in ANY regattas during the season.  Read instructions, then go to www.rowingwaiver.com and complete the waiver.
     

2009 AWAY Regatta forms:

  1. FCPS Parental Authorization and Acknowledgement of Risk for Field Trip    ALL rowers must fill out one (1) copy.  Use the following information to complete the form:

    • Date of trip: April 17-18, 2009

    • Destination: Mercer Lake, West Windsor, NJ

    • Supervision:  "directly supervised"

    • Transportation:  "commercial carrier"

    • Risk related:  "other" rowing

  2. FCPS Field Trip Luggage Search  ALL rowers must fill out one (1) copy for Mercer County Sprints.  Our departure date is April 17, 2009.
     

OTHER CREW FORMS:

Event Receipts Report   [Submitting funds to treasurer]  Please submit funds collected during a crew fundraising event using this form.

Expense Reimbursement Request   [Reimbursement for expenditures]  Please submit this form to treasurer for reimbursement for expenditures made on behalf of crew.
